ABSTRACT

Chronic rhinosinusitis (CRS) is a relatively common inflammatory disease of the nasal and paranasal sinus mucosa. Several epidemiological studies have established an association between particulate matter (PM) and CRS. Based on those data, PM has emerged as an important environmental factor in the development of CRS. Recent research has investigated the mechanisms and treatment options for CRS caused by PM through cellular experimentation. Therefore, the authors would like to explain the definition of PM, present research investigating the relationship between PM and CRS, and summarize the involved mechanisms reported to date.

ABSTRACT

@#AIM: To characterize a large cohort of patients presenting to the Department of Ophthalmology in Hunan Children's Hospital for ocular trauma, thus to offer supports for treatment and prevention of ocular trauma in children.<p>METHODS:The retrospective study was conducted between January 2016 and December 2020, and the medical recorded of 2 298 children(2 332 eyes)diagnosed with ocular trauma in the Ophthalmology Department and Ophthalmic Emergency of Hunan Children's Hospital were reviewed. The record data included gender, age, wounded eye, time of day, injury month, cause of injury, zone of injury, types of ocular injury, complications, method of treatment and prognosis of visual acuity. <p>RESULTS: The results showed that 44.26% of ocular trauma occurred in the age of 3-6 years old in children, and 1 718 cases(1 745 eyes)in male children versus 580 cases(587 eyes)in female children, with a male to female ratio of 2.96:1.00. The high incidence months were February and November for pediatric ocular injury. Collisions and falls were the main causes, accounting for 40.95%, following with blunt object injury and sharp object injury, accounting for 40.82%. Eyelid and other accessory organs were the main injury sites(65.40%). After injury, 58.62% of the children were treated with debridement, primary closure or topical administration of eye drops, 38.38% were treated with debridement and suturing, and 3.00% were treated with complex intraocular surgery. Most children patients were difficult to cooperate with the vision examination. Among the children who completed the vision examination, 60.60% showed visual impaired due to eye trauma. And after treatment, their vision improved to varying degrees. The severe impairment of visual acuity was mainly caused by corneal contusion, traumatic cataract, vitreous hemorrhage, bulbar foreign body and rupture of eyeball.<p>CONCLUSION: Pediatric ocular injuries are usually accidental, with an increasing incidence in recent years. The majority of children with ocular trauma are male and preschool children, with generally mild injuries and relatively good visual prognosis. Prevention and education on children's ocular traumas should be emphasized, and timely treatments after injury can effectively reduce the visual damage.

ABSTRACT

Objective:To investigate the clinical characteristics and therapeutic strategies of huge esophageal stones.Methods:One patient with esophageal esophageal calculus admitted to Beijing Friendship Hospital, Capital Medical University, who failed in undergoing endoscope treatment and shifted to an operation were retrospectively analyzed, and analyzed and summarized in combination with relevant literature.Results:The patient has significant dysphagia and severe malnutrition requiring further medical intervention. CT examination found that the lower esophagus was incarcerated with huge stones, and gastroscopy showed stones were obstructing the lumen, esophageal polyps and muscle bridges under the stones.Because the stones were large and hard, multiple endoscopic stone removal failed, and then the stones were successfully removed by thoracic surgery. The patient recovered well after surgery and had no complications such as anastomotic leakage or stenosis.Conclusions:Esophageal stones are easy to cause obstruction, and even lead to perforation, so it should be actively managed clinically. For patients with esophageal stones who have failed multiple endoscopic stone removals, thoracotomy can be used as a further treatment.

ABSTRACT

Trionycis Carapax is a commonly used Chinese medicine in clinical practice. Modern research on Trionycis Carapax mainly focuses on experimental research and clinical observation, which has been rarely reported in the literature. Based on the literature on medicinal herbs, medical books, prescriptions of all dynasties, this study carried out systematic textual research on the historical evolution and changes of the name, origin, producing areas, quality, efficacy, indications, processing methods, and contraindications of the Trionycis Carapax. As revealed by the textual analysis, the origin of Trionycis Carapax is Trionyx sinensis, and the carapace of T. steindachneri is not suitable for the preparation of Trionycis Carapax. The genuine producing areas of Trionycis Carapax include Yueyang, Jingzhou, southeast Anhui, and western Jiangsu in the middle and lower reaches of the Yangtze river. Regarding the quality, the number of ribs of Trionycis Carapax, such as seven ribs and nine ribs, is often used as the quality evaluation standard in ancient Chinese herbal books. However, through literature research and field inspections on the medicinal material markets, it is not advisable to take rib number as a quality evaluation criterion in modern times. With the change of the times, the efficacy and indications of Trionycis Carapax have gradually expanded on the basis of Shennong’s Classic of Materia Medica (Shen Nong Ben Cao Jing), and later generations widely apply it in internal medicine, surgery, gynecology, pediatrics, etc. It should be noted that the treatment of labor heat and bone steaming by Trionycis Carapax is derived from Shennong’s Classic of Materia Medica, not Treatise on the Nature of Medicinal Herbs (Yao Xing Lun) mentioned in ancient books such as Amplification on Materia Medica (Ben Cao Yan Yi). The processing methods of Trionycis Carapax are diverse, which are dominated by traditional vinegar processing. In terms of contraindications, Trionycis Carapax should not be compatible with bauxite and marble and is contraindicated in pregnant women. Those with spleen deficiency, weak stomach, and liver deficiency without heat should use it with caution. This study is expected to provide the basis for radical reform and further development and clinical utilization of Trionycis Carapax.

ABSTRACT

ObjectiveTo investigate the nephroprotective and anti-inflammatory effects of Fufang Shelong capsules (FFSL) in rats with membranous nephropathy (MN), and the role of the p38 mitogen-activated protein kinase (MAPK) signaling pathway. MethodMale SD rats of SPF grade were divided into a normal group and an experimental group. The MN model was induced by tail vein injection of cationized bovine serum albumin in the experimental group. After screening, the eligible model rats were included and divided into a positive control group (tripterygium glycosides tablets) and low-, medium-, and high-dose FFSL groups (0.375, 0.75, 1.5g·kg-1). The rats were treated correspondingly for eight weeks, and urine protein was detected during drug intervention. Renal function and inflammation-related indicators were detected after drug intervention. The changes in 24-hour urine total protein (24 h UP), interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α), creatinine (Cr), blood urea nitrogen (BUN), total protein (TP), albumin (Alb), and total cholesterol (TC) were detected. Flow cytometry was used to detect CD4+/CD8+ changes. Kidney tissues were collected to observe pathological changes under a light microscope and an electron microscope. The protein expression of p38 MAPK and phosphorylated p38 MAPK (p-p38 MAPK) in kidney tissues was detected by Western blot. ResultCompared with the normal group, the model group showed increased 24 h UP (P<0.01), elevated serum Cr, BUN, TC, IL-6, IL-8, and TNF-α (P<0.05,P<0.01), decreased serum Alb and TP levels (P<0.05,P<0.01), increased CD4+/CD8+ in the peripheral blood (P<0.01), and up-regulated protein expression of p38 MAPK and p-p38 MAPK in kidney tissues (P<0.05). Additionally, in the model group, immune complex deposition and foot process fusion, accompanied by infiltration of inflammatory cells, were observed on the epithelial side of the basement membrane in the pathological kidney tissues. Compared with the model group, the groups with drug intervention showed declining 24 h UP levels at six weeks (P<0.05,P<0.01), decreased serum Cr, BUN, TC, IL-6, IL-8, and TNF-α (P<0.05,P<0.01), increased serum Alb and TP levels (P<0.05,P<0.01), reduced CD4+/CD8+ in the peripheral blood (P<0.01), improved renal pathological damage, and down-regulated p38 MAPK and p-p38 MAPK in kidney tissues (P<0.05,P<0.01). ConclusionFFSL can decrease the expression of inflammatory factors, reduce proteinuria, delay kidney damage, and protect kidney function by inhibiting the expression of the p38 MAPK signaling pathway.

ABSTRACT

Esophageal cancer is one of the common malignancies of the digestive tract, and its exact etiology and pathogenesis have not been completely clear. The establishment of animal model of esophageal cancer provides support for the basic and preclinical research of esophageal cancer, which is of great significance for further study to clarify the pathogenesis and explore the development of new therapeutic drugs. At present, the animal models commonly used in the study of esophageal squamous cell carcinoma mainly include chemical induction model, heterotopic transplantation model and orthotopic transplantation model. With the continuous progress of animal research on human origin tumor, the human transplanted tumor model has been more and more widely used. No matter which model has its own advantages and disadvantages and applicability, should be selected according to the purpose of the experiment. In this paper, the research progress on animal models of esophageal squamous cell carcinoma in recent years is reviewed, and the models commonly used in scientific research and preclinical treatment are discussed, which may provide a theoretical basis for the clinical treatment.

ABSTRACT

Malignant melanoma is a kind of tumor produced by human melanocytes. It has a high degree of malignancy, early metastasis and high mortality. Most melanomas are caused by malignant skin nevus and can also be seen in the digestive tract such as rectum and anus. Primary malignant melanoma of pleura is rare, rarely seen in case reports. This paper reports the clinical data of a case of malignant melanoma with cough, expectoration and pleural effusion as the first symptoms diagnosed by thoracoscopy combined with pathology in Beijing Friendship Hospital Affiliated to Capital Medical University, and analyzes and summarizes the literature data.

ABSTRACT

Macleaya cordata, a perennial plant in the genus Macleaya, it mainly distributed in most provinces and regions south of the Yangtze river and north of Nanling mountains of China and has been used as a Chinese traditional medicine. It is bitter, cold, warm, poisonous. M. cordata has the functions of dispelling wind, analgesia, detoxification and detumescence. It mainly treats poisonous abscess, cachexia, ulcer, scabies, trichomonal vaginitis, etc. It also has insecticidal and anti-itching activities. The main chemical constituents of M. cordata are isoquinoline alkaloids, sanguinarine, chelerythrine, protoopioid and allocrine alkaloids are the higher ones. In addition, it also contains phenylpropanoids, steroids, organic acids, phenols and volatile oils. The pharmacological effects of M. cordata are mainly anti-bacterial, anti-inflammatory, anti-tumor and improving liver function. In agriculture, it can be used as botanical insecticides and bacteriostasis, and also as feed additives for animal husbandry. By reviewing and analyzing domestic and foreign researches that isoquinolines were the main active constituents and characteristic components of M. cordata. This paper provides theoretical basis for the development and utilization of M. cordata extract and its' monomer compounds.

ABSTRACT

BACKGROUND@#Pulmonary vein (PV) occlusion generally depends on repetitive contrast agent injection when cryoballoon ablation for atrial fibrillation (AF). The present study was to compare the effect of cryoballoon ablation for AF guided by transesophageal echocardiography (TEE) vs. contrast agent injection.@*METHODS@#Eighty patients with paroxysmal AF (PAF) were enrolled in the study. About 40 patients underwent cryoballoon ablation without TEE (non-TEE group) and the other 40 underwent cryoballoon ablation with TEE for PV occlusion (TEE group). In the TEE group during the procedure, PVs were displayed in 3-dimensional images to guide the balloon to achieve PV occlusion. The patients were followed up at regularly scheduled visits every 2 months.@*RESULTS@#No differences were identified between the groups in regard to the procedure time and cryoablation time for each PV. The fluoroscopy time (6.7 ± 4.2 min vs. 17.9 ± 5.9 min, P < 0.05) and the amount of contrast agent (3.0 ± 5.1 mL vs.18.1 ± 3.4 mL, P < 0.05) in the TEE group were both less than the non-TEE group. At a mean of 13.0 ± 3.3 mon follow-up, success rates were similar between the TEE group and non-TEE group (77.5% vs. 80.0%, P = 0.88).@*CONCLUSIONS@#Cryoballoon ablation with TEE for occlusion of the PV is both safe and effective. Less fluoroscopy time and a lower contrast agent load can be achieved with the help of TEE for PV occlusion during procedure.

Objective@#A label-free electrochemical immunosensor was developed for the detection of nuclear matrix protein-22 (NMP22) as a biomarker of bladder cancer.@*Methods@#The study was based on the establishment and validation of the methodology. Urine samples were collected from 20 patients with bladder cancer and 20 controls in the affiliated Hongqi hospital of Mudanjiang medical university from September in 2017 to July in 2019 to validate the developed method. A screen-printed electrode (SPE) was modified with a film of a composite made from the reduced graphene oxide-tetraethylene pentamine (rGO-TEPA) immobilized Zn-based-Metal-organic frameworks deposited with Au nanoparticles (rGO-TEPA@Au-ZIF8). Primary antibody against NMP22 was immobilized on the Au nanoparticles on the surface of the modified SPE, which then was blocked with bovine serum albumin to elimiate nonspecific binding sites. The process of the construction of the proposed sensorwas characterized by cyclic voltammetry and electrochemical impedance spectroscopy. Differential pulse voltammetry was used to evaluate the linear range, recovery, precision, selectivity and stability. The data were analyzed by Mann-Whitney U test.@*Results@#Under optimal conditions, the immunosensor exhibited a linear range of 0.01-1000 ng/mlwith a detection limit of 3.33 pg/ml (S/N=3) and a standard recovery of 97.65%-107.05%. The levels of NMP22 in urine samples from patients with bladder cancer [66.03 (4.34, 91.74)]ng/ml determined by the proposed sensor were significantly higher than those of controls 0.54(0.06, 8.84) ng/ml(P=0.001).@*Conclusion@#The immunosensor can achieve sensitive, rapid and acucurate detection of NMP22, and has potential application prospects in monitoring tumor markers.

ABSTRACT

<p><b>OBJECTIVE</b>To explore brain activations associated with electroacupuncture simulation at Tongli (HT 5) and its comparison with brain activations during picture-naming task.</p><p><b>METHODS</b>Twenty healthy subjects were enrolled in this study. Half of them received electroacupuncture stimulation at HT 5 (ACUP group) and the other half of them received stimulation at a nonmeridian sham acupoint (SHAM group). All subjects performed picture-naming task. Each subject finished two runs of functional magnetic resonance imaging examinations in one session and picture-naming task was performed before electroacupuncture stimulation. Subjective brain activations were obtained using generalized linear model and inter-group analyses were performed after that.</p><p><b>RESULTS</b>The electroacupuncture stimulation at HT 5 induced significant brain activations in both the anterior and posterior language regions, including the left inferior frontal gyrus, which was in consistent with activations induced during picture-naming task. Group analysis showed a tendency of increased activation of ACUP group in left inferior frontal gyrus compared with SHAM group (P<0.05 FDR corrected).</p><p><b>CONCLUSIONS</b>Electroacupuncture treatment at the acupoint HT 5 has modulation effect on typical language-implicated brain regions in healthy subjects, which provides supporting evidence for beneficial effects of needling at HT 5 for recovery of language function in aphasia.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of different operation time for acute spinal cord injury(SCI) based on systematic review.</p><p><b>METHODS</b>PubMed database, EMBASE database, Cochrane Library, ISI Web of knowledge, CBM database, VIP database, CNKI database and Wanfang database were searched from their start year up to February 2017 for relevant randomized clinical trials on the treatment of acute spinal cord injury with different intervention times.</p><p><b>RESULTS</b>Four randomized clinical trials of total 156 cases were included. Early surgical intervention for the patients with incomplete spinal cord injury can improve the ASIA motor function score [MD=3.29, 95%CI(-7.90, 14.49), =0.56] and overall Frankel score[=7.65, 95%CI(2.69, 21.74), =0.000 1]. There was no significant difference in the improvement of the overall Frankel score[=4.88, 95%CI(0.74, 32.09), =0.10] for the patients with complete spinal cord injury between the early surgery and delayed surgery group. There was no significant difference in hospitalization time[MD=-3.4, 95%CI(-8.12, 1.32), =0.16], death rate [=1.07, 95%CI(0.21, 5.56), =0.93]and incidence of decubitus[=1.07, 95%CI(0.17, 6.69), =0.94] between the early surgery and delayed surgery group.</p><p><b>CONCLUSIONS</b>Early surgical intervention can promote the nerve function recovery after spinal cord injury, whithout further incidence of complications, but random control trails with higher quality are still required for this conclusion.</p>

ABSTRACT

Objective To analyze the efficiency of perinatal group B streptococcal(GBS)infection preven-tion in the local area.Methods From June 2015 to June 2016,3 667 pregnant women were included.Both pre-natal examinations and deliveries were done in our hospital.The analysis of GBS colonization,risk factors and prevention of early-onset disease were done.Results Among preterm and term pregnant women,the rates of GBS screening were 23.4% and 35.6%,respectively.The positive results of GBS were 10.9% and 8.4%,re-spectively.The percentage of women with 2 risk factors were 22.9% and 0.3%,respectively.GBS screening has a better prediction effect for GBS colonization status of women at labor.There were significant differences in the intrapartum antibiotic prophylaxis(IAP)implementation between preterm and term pregnant women. In the IAP efficiency analysis of preterm,GBS colonization and risk factors,preterm had a high sensitivity (96.1%),while the specialties of GBS carry and risk factors were high(93.2% and 90.3% respectively).Con-clusion preterm pregnant women should be highlighted as an important population in the implementation of IAP.GBS screening strategy is better than the risk factors strategy.

ABSTRACT

Objective To preparegraphene quantum dots (GQDs) and construct a novel biosensor for determination of dopamine (DA) and uric acid (UA).Methods The GQDs was prepared by carbonization method from citric acid as carbon sources.Differential pulse voltammetry was used by the modified electrodes to detect uric acid and dopamine,and the detection performance was evaluate.Collected three experimenters morning urine on July 29,2016.The proposed sensor was used for biological samples detection.Results The size of GQDs were between 3 to 5 nm,which was observed by transmission electron microscopy (TEM).The proposed sensor had good linear correlation of 50~600 μmol/L UA (r2 =0.996 6) and 2~ 240 μmol/L DA (r2 =0.992 5).In uric acid in urine samples' detection (n=3),RSD value was less than 3%.The standard addition recovery rates of UA and DA were in 95.7%~101.4% and 96%~102% respectively.Conclusion The biosensor based on GQDs for determination DA and UA had been constructed successfully.

ABSTRACT

Objective@#To compare the outcome of radiofrequency catheter ablation under local anesthesia/sedation (S) or general anesthesia(GA) in atrial fibrillation patients.@*Methods@#Data of 498 patients with atrial fibrillation undergoing radiofrequency catheter ablation in our departmentfrom January 2014 to December 2015 were retrospectively analyzed. Two hundred and twenty patients assigned to the GA group, the other 278 patients to the S group. Patients were followed clinically every 3 months within one year after procedure. Immediate electrocardiogram was performed in patients with palpitation or choking sensation in chest. The end point of the study was recurrence of any atrial tachyarrhythmia lasting >30 seconds in device interrogation, 24-hour Holter monitoring or 12-lead electrocardiogram after a single procedure. After the ablation procedure, a blanking period of 3 months was allowed according to the guidelines. Procedure time, radiofrequency time, fluoroscopy time, the detection of paroxysmal supraventricular tachycardia, the success rate and the complications were compared between the two groups.@*Results@#There was no difference in the baseline characteristics between the two groups, such as age, gender, BMI, complications, LVEF, LAD (all P>0.05). The duration of procedure ((117.8±51.7)minutes vs.(115.4±36.9)minutes, P=0.79), duration of fluoroscopy((12.5±11.2)minutes vs. (10.4±10.2)minutes, P=0.35), duration of radiofrequency((40.1±12.9)minutes vs. (48.6±44.3)minutes, P=0.48) were similar between the two groups (P>0.05). Compared with S group, discovery of the frequency of atrioventricular node reentrant tachycardia (AVNRT) was significantly lower in GA group (0 vs. 3.6%(10/278), P<0.01), but the difference disappeared with repeat electrophysiological examination when patients become conscious from GA(3.2%(7/220) vs. 3.6%(10/278), P=0.311). The difference of atrioventricular reentrant tachycardia (AVRT) was similar between the two groups(0.9%(2/220) vs. 0.7%(2/278), P=0.841). Compared with S group, reflection of vagus nerve was less in GA group (1.4%(3/220) vs. 8.6%(24/278), P=0.026). After following up of (356±92) days, freedom from atrial fibrillation/atrial flutter/atrial tachyarrhythmia was similar between the two groups(77.9%(162/208) vs. 79.9%(215/269), P=0.818).@*Conclusion@#General anesthesia is a promising method to atrial fibrillation ablation, in view of stable patient status and safety for the procedure. There is no difference in complications, recurrence of arrhythmia between the two groups, but detection rate of AVNRT is lower in GA group.

ABSTRACT

Objective To analyze the correlation between the pressure volume parameters and cardiac function in terms of New York Heart Association(NYHA) classification in patients with pulmonary arterial hypertension. Methods Among 36 patients with pulmonary hypertension admitted in our center between April 2015 to June 2016, right heart catheterization recording right atrial pressure curve,right ventricular pressure curve,pulmonary arterial pressure and oxygen saturation curve in different parts was performal.All patients recived underwent cardiac MRI examination to obtain a single cardiac cycle and the use of Mass software to measure right ventricular volume continuously and right ventricular pressure-volume loop parameters were then obtained. Patients were divided into different groups according to the NYHA functional classification, and the correlation between the parameters of each group and the cardiac function classes a were analyzed. Results Compare to patients with NYHA class Ⅰheart function,patients with NYHA heart function class Ⅱ and class Ⅲ had significantly higher right ventricular end diastolic pressure(P < 0.05)and higher right ventricular systolic pressure (P < 0.01). Spearman correlation analysis showed that cardiac function of NYHA classⅠ,Ⅱ and Ⅲhad position correlation with RVESV,RVEDP and RVESP wheras negative correlation with RVEDV. ROC curve analysis showed that,when the patients were divided into 2 groups as NYHAⅠvs. NYHA class Ⅱ + Ⅲ,NYHA classification for predicting the outcome of the NYHA class Ⅱ + Ⅲ level, pulmonary artery elasticity and right ventricular end systolic pressure had larger area under curves respectively. Conclusions Pressure-volume parameters of right ventricles are more objective indicators for cardiac function assessment for pulmonary hypertension patients and evaluation of disease progression especialy in patients with mild symptoms.

ABSTRACT

In total hip arthroplasty (THA), the accurate positioning of implants is the key to achieve a good clinical outcome. Computer-assisted orthopaedic surgery (CAOS) has been developed for more accurate positioning of implants during the THA. There are passive, semi-active, and active systems in CAOS for THA. Navigation is a passive system that only provides information and guidance to the surgeon. There are 3 types of navigation: imageless navigation, computed tomography (CT)-based navigation, and fluoroscopy-based navigation. In imageless navigation system, a new method of registration without the need to register the anterior pelvic plane was introduced. CT-based navigation can be efficiently used for pelvic plane reference, the functional pelvic plane in supine which adjusts anterior pelvic plane sagittal tilt for targeting the cup orientation. Robot-assisted system can be either active or semi-active. The active robotic system performs the preparation for implant positioning as programmed preoperatively. It has been used for only femoral implant cavity preparation. Recently, program for cup positioning was additionally developed. Alternatively, for ease of surgeon acceptance, semi-active robot systems are developed. It was initially applied only for cup positioning. However, with the development of enhanced femoral workflows, this system can now be used to position both cup and stem. Though there have been substantial advancements in computer-assisted THA, its use can still be controversial at present due to the steep learning curve, intraoperative technical issues, high cost and etc. However, in the future, CAOS will certainly enable the surgeon to operate more accurately and lead to improved outcomes in THA as the technology continues to evolve rapidly.
